- Evaluates the needs of the member, barriers to accessing the appropriate care, social determinants of health needs, focusing on what the member identifies as priority and recommends and/or facilitates the plan for the best outcome
- Develops ongoing care plans / service plans and collaborates with providers to identify providers, specialists, and/or community resources to address member's unmet needs
- Identifies problems/barriers to care and provide appropriate care management interventions
- Coordinates as appropriate between the member and/or family/caregivers and the care provider team to ensure members are receiving adequate and appropriate person-centered care or services
- Provides ongoing follow up and monitoring of member status, change in condition, and progress towards care plan / service plan goals; collaborate with member, caregivers, and appropriate providers to revise or update care plan / service plan as necessary to meet the member's goals / unmet needs
- Provides resource support to members and care managers for local resources for various services (e.g., employment, housing, participant direction, independent living, justice, foster care) based on service assessment and plans, as appropriate
- Facilitate care management and collaborate with appropriate providers or specialists to ensure member has timely access to needed care or services
- May perform telephonic, digital, home and/or other site outreach to assess member needs and collaborate with resources
- Collects, documents, and maintains all member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators
- Provides and/or facilitates education to members and their families/caregivers on disease processes, resolving care gaps, healthcare provider instructions, care options, referrals, and healthcare benefits
- Provides feedback to leadership on opportunities to improve and enhance care and quality delivery for members in a cost-effective manner
- Other duties or responsibilities as assigned by people leader to meet business needs
- Performs other duties as assigned

***This is a remote position. Ideal candidate will be located in the Eastern Standard Time (EST) zone with a compact registered nurse (RN) license, experience in case management, or medical-surgical (med surg), intensive care unit (ICU) settings and leadership experience, either as a leader or in a lead role, in a clinical setting. Having proficiency in a second language (bilingual) will be another great as asset.
Education/Experience: Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience. License/Certification:
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required

- The RN is responsible for awareness of the physical, emotional and spiritual needs for a group of residents and coordinating/providing that care within the scope of practice and policies and procedures of the Community Living Center; the number of residents differs on each shift.
- The RN will oversee and supervise several nursing assistants and LPN's to ensure the patient care is delivered timely and accurately.
- The RN will work alongside their nursing care team to ensure the care is delivered; physical, emotional, and spiritual
- Ensure medications are administered timely and accurately, and report medication errors promptly and timely.
- The RN works together with the Interdisciplinary Team to ensure everything possible is done according to the resident's wishes.
- The RN will ensure daily operations of the neighborhood are carried out smoothly.
- The RN will communicate with NM or supervisor any unusual issues, staffing needs, resident incidents
- The RN will identify needs for education of care partners they oversee and either provide or coordinate the provision of that education in a timely manner.

Qualifications
- English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware ( ICD ).
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)

Nurse I, Level III | Demonstrates proficiency in practice based on conscious and deliberate planning. The North Florida/South Georgia Veterans Health System consists of two medical centers, three large multi-specialty outpatient clinics and eight small community-based primary care outpatient clinics. The Malcom Randall VA Medical Center in Gainesville is a tertiary care facility, identified as the most complex VA health care system, that is also an active teaching hospital, with an extensive array of specialty services. It combines a full range of patient care services with state-of-the-art technology that is enhanced and supported through education and research. We have a strong and meaningful affiliation with the University of Florida College of Medicine and are connected both physically and functionally to the University. We train over 2,000 residents, interns, and students each year. Other affiliations are in place with Florida State University, Valdosta State University, Santa Fe Community College, and Lake City Community College. Through sharing agreements, there is collaboration in the areas of radiation therapy, professional radiology services, sleep lab, electron microscopy, and cardiac catheterization
