M.D.-Ph.D. and M.D. Degrees, Salaries and What They Really Cost

Whether you're obtaining a Ph.D. or an M.D. degree in medicine, it is no easy undertaking . Years of intensive study, research and internships yield a degree that not only allows you to put those two little letters in front of your name, but usually comes with an impressive starting salary as well. However, what is the difference between an M.D.-Ph.D. and an M.D.?

The difference between the degrees

Before you compare salaries, you should understand the difference between the degrees themselves. Someone who receives an M.D. is licensed to practice medicine and participate in patient care. Someone who receives an M.D.-Ph.D. works in research and development, such as biomedical research, immunology or biochemistry, etc., but can also practice clinical medicine. A nonmedical Ph.D. can be in just about any subject, including the sciences, and varies as widely in cost and salary as it does in subject matter. For the purposes of this cost and salary comparison, we'll stick to M.D.-Ph.D.'s and M.D.'s.

You also must make sure you compare apples to apples. Before taking salary into consideration, measure other factors, such as the cost of each degree and subsequent school debt , living expenses, and potential stipends and funding offered.

According to the AAMC, the average cost of four years of medical school is $228,006 (that's averaging together public vs. private schools and resident vs. nonresident). Of course, that's not taking into consideration the cost of living and other expenses, which will vary depending on where you go to school, for example, New York City, N.Y. vs. Bloomington, Ind.

While physicians shoulder some of the highest school debt of any profession, they also enjoy some of the highest salaries, which is helpful when they are making mortgage-sized payments for their school loans. The national average for annual physicians' salaries in the U.S. is $209,044. However, not all costs to train a medical doctor fall on the medical student. The costs that a university and society expend on each medical student are fairly exorbitant as training excellent medical professionals is highly valued in this country.

While students pay for medical school, the university hospital foots the bill for the student's residency, which includes expenses such as hospital call rooms, administrative costs and salaries for the student and supervising faculty. For specialties that take longer to train for, the costs are in excess of $1,000,000. Where tax paying citizens come in is that most university hospitals receive federal funding, some of which is used to pay for students' residencies.

M.D.-Ph.D. costs

Falling into the category of medical scientists, M.D.-Ph.D. recipients are trained in both clinical medicine and research. This type of dual degree is ideal for conducting clinical trials, as both intensive research skills and patient care involvement are required. Because this is a dual degree, the time it takes to complete it is considerably longer than the four years for just medical school. An M.D.-Ph.D. typically takes seven to eight years to complete due to the student having to attend both medical and graduate schools.

The major difference between an M.D. and an M.D.-Ph.D. is the cost of tuition. While medical school tuition and costs will run well into six figures over four years, M.D.-Ph.D. students will find that their costs are largely reduced or even eliminated through waivers and stipends.₁ The National Institute of General Medical Sciences currently funds forty-nine M.D.-Ph.D. programs through the Medical Scientist Training Program. 2

Many universities also offer full funding to M.D.-Ph.D. candidates as they leverage the exorbitant cost of educating these students against the expected scientific breakthroughs and accomplishments they'll make in the medical field. The state of health care and the increasing need to study and treat numerous diseases affecting the population have contributed to a faster than average job growth rate for medical scientists. 3

Salary is a different story, as it's very dependent on what type of work the M.D.-Ph.D. pursues. Working in just the scientific research field will yield a median annual salary of $91,510, considerably less than the mid-six-figure salaries of practicing physicians; however, those who practice medicine as well as medical research will see their salaries increase accordingly.

The application process

Both degrees carry a high level of competitiveness, and the application process can be an intensive one. There are no test-optional choices here. MCATHs are a given requirement , so start preparing as soon as you've made the decision to apply to these programs.

Submitting a strong and effective statement of purpose , personal statement and letters of recommendation are critical to getting that coveted acceptance letter from your choice schools. It’s important to keep track of what you've submitted and when, as well as deadlines and communications with program recruiters or advisors.

Choosing your path

Entering the medical field, in any capacity, is a noble and stressfully intensive undertaking. However, the effect you'll have on patients' lives or future treatments and cures for diseases makes the investment of the student, university and government well worth it. Dietitians and nutritionists educate clients on nutrition and help them adopt the right diets for their health needs. Faculty salaries increased 2.7% in 2018

The aamc’s annual faculty salary report highlights mean salaries across departments and specialties, academic ranks, and degrees in all regions of the country..

The AAMC Faculty Salary Report, FY 2018 includes much more detailed salary information, including mean compensation by type of medical school and region for each combination of six faculty ranks and 92 specialties. Total compensation included fixed salary plus supplemental earnings, which includes medical practice earnings, bonus pay, and outside earnings such as speaking engagements and consulting fees.

Online access to the report is available to individuals at member institutions for an annual subscription fee of $40 per year and to nonmember institutions for $1,125 per year through the AAMC Store . The printed edition will be available for purchase in early 2019. The print edition will include the most recent year of compensation data, while the online version includes three years of salary data.

MSL Salary and Compensation: 2022 Data from Medical Science Liaisons and MSL Management Roles in the United States

Salary and compensation benchmarking are essential factors that lead to successful recruitment and retention of MSL talent. Companies must have a clear understanding of MSL salary and compensation levels across different Therapeutic Areas/disease states to remain competitive.

To gain a more accurate understanding of MSL salary and compensation levels globally, the MSL Society conducted the 9th MSL Salary and Compensation Survey in late 2022, which included 2,099 MSL professionals from 60 countries. The primary purpose of the survey was to gain insights into current global MSL salary and compensation levels across pharmaceutical, biotechnology, medical devices, and other healthcare companies that employ MSLs and MSL leaders.

The MSL Society conducted the first-ever MSL salary survey in 2014, and as in previous years, the 2022 report serves as the largest database of MSL salaries. As such, it remains the most comprehensive and authoritative resource on global MSL salary and compensation levels.

Individual country reports were created for all countries that received a sizeable number of responses. Based on the data gathered data, the 2022 Salary & Compensation Survey is available in 15 versions, including the overall global results and 14 individual country-level reports: Australia, Brazil, Canada, Colombia, France, Germany, India, Italy, Mexico, Portugal, Spain, Turkey, U.K., and USA. However, this article will focus only on the U.S. results (all other reports and data are available on the MSL Society website).

The 2022 MSL Salary & Compensation survey included 2,099 MSL professionals from 60 countries, and the data was collected from an online survey that was open from August 25th to December 13th. The survey was only open to current MSLs (or equivalent titles), MSL management, MSL Excellence/Operations, and Executive Management. Respondents were only allowed to participate once and duplicate surveys from a single email address were not accepted. The survey results were not weighted, and partial responses that included salary data were included in this report, meaning that the N value in some tables will not add up to the total report N.

Respondents were invited to participate in the survey through a range of sources, including: 

  • Announcements in our LinkedIn group “Medical Science Liaison & Medical Affairs Networkers,” as well as other LinkedIn groups
  • The MSL Society newsletter
  • Personal direct LinkedIn contacts

The U.S. data includes 1,384 current MSL professionals consisting of 1,032 Medical Science Liaisons (MSLs), 35 MSL Excellence/Operations, 219 Manager/Director of MSLs, 65 Executive Management/Vice President of Medical Affairs, and 33 identifying as Others. When split by gender, 42% of respondents were male, 54% female, and 4% identified as other. The U.S. data includes MSL professionals from all company types and diverse educational backgrounds with a wide range of years of experience. The salary calculations in all Tables in the survey represent only base salaries and do not include other compensation. 

The respondents also identified their company type, in which pharmaceutical/biotechnology companies represented 87% of all U.S. participants, subdivided into 36%, 23%, and 28% representing Large, Medium, and Small Pharmaceutical/Biotechnology companies, respectively. The remaining respondents were Medical Devices, Contract Research Organizations, Diagnostic Companies, Contract MSL Organizations, and Others.

AVERAGE SALARY BASED ON COMPANY TYPE

Based on the data, company type, plays a role in the compensation rates of MSL professionals. On average, MSLs and MSL managers that work for Large, Medium, and Small Pharmaceutical/Biotechnology companies have the highest salaries. On average, executives working for Large, Medium, and Small Pharmaceutical/Biotechnology companies and CRO’s have the highest salaries. (See Tables 1-4.)

AVERAGE SALARY BASED ON YEARS OF EXPERIENCE

As in previous annual reports, the 2022 report revealed (as expected) that generally, one’s salary within the MSL profession steadily increases with years of experience. However, interestingly, there are instances where, on average, the salary for both MSLs/Sr. MSLs and Managers/Directors decrease unexpectedly. (Tables 5-8) .

SALARY SATISFACTION 

Companies must ensure that the MSL professionals they employ receive competitive salaries to strengthen retention. According to the 2022 survey results, more than 89% of MSLs reported being “satisfied” or “very satisfied” with their overall annual compensation. In similar satisfaction rates, 76% of Manager/Director of MSLs, reported they are “satisfied” or “very satisfied” with their overall annual compensation. Those in Executive Management/VP of Medical Affairs and MSL Excellence/Operations also reported high satisfaction rates with their overall compensation. (Tables 9-12.)

SALARY TRANSPARENCY

Importantly, the 2022 survey also included questions about salary transparency. Results reveal different perspectives regarding transparency. Interestingly, although 79% of MSLs indicated that salary information for their role should be public information at their company, 56% of Managers/Directors and 72% of Executive Management do not think this information should be publicly available. Another finding was that 88% of MSLs and 62% of managers indicated that salary details should be included in all job postings for roles within Medical Affairs. (Tables 13-16.)

Dr. Dyer has also written extensively on the Medical Science Liaison role, including numerous published articles, benchmark studies, and reports. Dr. Dyer is well recognized within the global MSL community and has developed an extensive international network within the Pharmaceutical, CRO, Medical Device, and Biotechnology industries. He is the owner of the largest group on LinkedIn for MSLs and Medical Affairs with over 25,000 members. He has spoken and moderated several international conferences on various MSL topics including KOL management, creating MSL teams, MSL training, international MSL teams, and the value of the MSL role and Medical Affairs. Dr. Dyer is consistently sought out as a resource and consultant for MSL projects that have included diverse companies such as McKinsey Consulting, Bain and Co., and Philips Healthcare.

Dr. Dyer has a Ph.D. in Health Sciences and did medical training in Chicago. He has a Master’s Degree in Tropical Biology (where he studied in the Amazon) and a B.S. in Biology. Prepare Yourself for a Successful Medical Career

SCU’s post-baccalaureate Online or Blended Master of Science in Medical Science (MSMS) degree is designed to assist students who would like to enhance their academic credentials for entry into medical, dental, physician assistant, veterinarian, or other professional healthcare programs. Improve your academic portfolio with a wealth of foundational health sciences knowledge and create new pathways to your ideal healthcare career. The program of study mirrors the traditional first year of medical school in its design, pace, and rigor and prepares you for success in your first year of medical school.

Flexible Options

Students may opt for one of three program schedule and delivery method combinations, each of which offers the same courses and degree. 11-month blended (on-campus) , 11-month 100% online , or 20-month 100% online . SCU doesn’t specify whether you earn your degree online or in person. All diplomas and transcripts simply say “Southern California University of Health Sciences.” That’s because SCU online students learn from the same faculty and receive the same course content as in-person students receive.

Post-MSMS Fellowship

Graduates of the MSMS program are eligible to apply. Fellows help teach courses and conduct research. SCU provides funding for both coursework and extracurricular projects.  The 11-month part-time position gives two Fellows the opportunity to receive further education, research experience, and professional academia experience as MSMS Moderators. Flipped Classroom Design

A flipped classroom design is an innovative approach to teaching. In a flipped classroom setting, you watch lectures online and completing assigned reading before class.  During class, your time is dedicated to i mmersive discussions, interactive cases, and collaborative group work. You also have access to pre-recorded lectures given by MSMS faculty on concepts and learning material critical for in-class sessions and examinations.

Spend time working through clinical scenarios, allowing you and your classmates to experience firsthand how your education can be applied to actual healthcare delivery.

In as little as one year, you can improve your academic record, increase your Medical College Admissions Test (MCAT) scores, and gain a competitive advantage for applying or re-applying to medical school, dental school, physician assistant programs, or other competitive health professional schools. Our MSMS graduates are eligible for guaranteed medical school interviews with our partner institutions. Partner medical schools include Ponce Health Sciences University, Ross University School of Medicine, American University of the Caribbean School of Medicine, American University of Antigua, and A.T. The classes in the MSMS program mirror the curriculum of the first year of LCME-accredited medical school, so students can prepare for what’s next after graduation. This first year of learning is an excellent foundation for all students. Both full-time and part-time students follow the same curriculum, the only difference is the number of credits taken each term. Students receive frequent one-on-one faculty advising throughout the program, regardless of the model chosen.

Students will take a comprehensive exam at the end of select terms to help assess progress.  After completion of the program, students are additionally provided the NBME Comprehensive Basic Science Self-Assessments (CBSSA). This exam gives our students an advantage by providing them with an evaluation of their readiness for the USMLE®.

11-month Blended The 11-month blended on-campus program utilizes multiple learning environments. Students participate in on-campus virtual synchronous instruction Monday through Friday from approximately 7:00 am to 1:00 pm PT. During this time students have virtual lectures, self-directed learning time, examinations, and interwoven small group discussions.

11-month Online The 11-month 100% online courses are offered Monday through Friday, generally in 5- to 6-hour blocks, through virtual interactive sessions delivered online by faculty via live video feed. Students are highly encouraged to refrain from full-time employment due to the rigorous format of the program. This program is designed for students who are aiming for clinical, not research, careers.

20-month Online For students interested in flexible on-demand learning, this program can integrate with your schedule and classes can be taken from anywhere in the world. Students take the same curriculum, however, courses are spread over five terms instead of three for a flexible course load. Courses are offered online Monday through Friday. Recorded lectures are consumed outside of class flexibly and on-demand, while the weekly synchronous class time is focused on student comprehension. One to two hours of scheduled class time are required per week with two date/time options to choose from.

I chose the MSMS program because I am an inspiring physician, and this program focuses on the rigor of what it would be like for the first year of medical school. As a first-generation student, I wanted to be more competitive when applying to medical school, and this program helped me achieve that. My favorite course in the program was the Neuroscience course; Dr. Green and Dr. Nygard were the best instructors. They made this challenging course easier to comprehend. I liked that they focused on the clinical manifestations because I am interested in that area as an inspiring physician. As a CNA and a Nurse Assistant, I could see how a neurologist would perform these neurological exams on patients, so it was extraordinary for me to learn about it and conduct it.

Applicants to the Master of Science in Medical Science must meet the following minimum requirements. Applicants who do not meet all standards may be admitted with the approval of the Program Director; exceptions are made on an individual basis.

A completed bachelor’s degree from an accredited institution is required. While applicants typically have degrees in biology, biochemistry, chemistry, physics, or other related fields, applicants with degrees in other fields of study will be considered.

Undergraduate GPA of 2.7 on a 4.0 scale. Exceptions to the GPA requirements are considered on a case-by-case basis. Prerequisite Entrance Requirements Complete all required prerequisites with a C or better. Exceptions to prerequisite requirements may be considered on a case-by-case basis. Applicants from institutions following the quarter system will have an equivalent semester credit number calculated for prerequisite requirements. Please note that medical school or other health professional schools may require Physics I & II as a prerequisite for satisfying admissions requirements. Students who are missing a prerequisite course requirement or are looking to improve their GPA can choose to take an SCU Accelerated Science course and complete the requirements in as little as five weeks. Required Prerequisites

24 semester hours (36 quarter units) of life and physical science courses with a grade of C or higher. Exceptions to prerequisites requirements may be considered on a case-by-case basis. Please note that medical schools or other health professions programs may require additional courses as prerequisites to satisfy their admissions requirements; please refer to those programs for their admission prerequisites.

  • Minimum of 8 semester hours (12 quarter units) of biology, such as general biology, physiology, anatomy, and either genetics or cell biology
  • Minimum of 8 semester hours (12 quarter units) of Chemistry
  • Minimum 8 semester hours (12 quarter units) of Organic Chemistry

Recommended Prerequisites (not required)

  • Physics – minimum of 8 semester hours (12 quarter units)
  • Mathematics – 6 semester hours (9 quarter units)
  • Biochemistry – 6 semester hours (9 quarter units)

Test scores from one of the following standardized admissions tests are recommended but not required: MCAT, DAT, OAT, PCAT, GRE, or VCAT.

Personal Statement

In some circumstances, applicants may be asked to submit a 300-600 personal statement explaining reasons for pursuing the Master of Science in Medical Science, short-term and long-term career plans, and how background, previous experience, interest, and/or values, when combined with a Master of Science in Medical Science degree from Southern California University of Health Sciences, will enable successful pursuit of these goals. Southern California University of Health Sciences (SCU) has announced a new partnership with A.T. Still University-School of Osteopathic Medicine in Arizona (ATSU-SOMA) to expand access to education for aspiring physicians looking to earn a Doctor of Osteopathic Medicine (DO) degree. Through this partnership, five annual interview slots are reserved for SCU students who graduate from the Master of Science in Medical Science (MSMS) program for consideration of acceptance to the Doctor of Osteopathic Medicine degree program at A.T. Still University. The two-year agreement began in January 2023 and will be reviewed every two years.

Since 2021, the Master of Science in Medical Science (MSMS) degree program at Southern California University of Health Sciences (SCU) has partnered with the RAND Center for Collaborative Research in Complementary and Integrative Health.

American University of Antigua College of Medicine (AUA) is an innovative medical school dedicated to providing a learner-centric education of the highest quality, granting opportunities to underrepresented minorities, fostering a diverse academic community, and ensuring that its graduates develop the skills and attitudes of lifelong learning, compassion, and professionalism. Post-MSMS Fellowship/Moderator Program

The SCU Master of Science in Medical Science (MSMS) program offers a Post-MSMS Fellowship/Moderator program allowing two SCU MSMS students to apply for the 11-month fellowship, including moderator activities. The MSMS Fellowship is a part-time position allowing Fellows to receive further education, research experience, and professional academia experience as MSMS Moderators.

About the position:

With a growing population, one of the greatest challenges is ensuring that healthcare we receive is at the best possible quality for all across all dimensions of society – across all genders, ages, races, and ethnicities and socioeconomic levels.

The MSMS Fellowship/Moderator program is based on an interdisciplinary, integrative healthcare model looking at whole-person care, including healthcare practice from the communities where we live, impact of disease based on the lifestyles we practice such as the food we eat, the transportation we depend on to access healthcare facilities, and the community health services we receive. Success in the interdisciplinary healthcare team will require the translation of high quality and high impact science and practical clinical experience into sound health policy. The goal of the MSMS Fellows/Moderator Program is to provide MSMS students with increased opportunities, experience and skills necessary to help lead this effort, and in so doing, shape a healthy and productive future for all.

The MSMS Fellowship has an emphasis on teaching courses and conducting research and provides funding for both coursework and extracurricular projects.

Moderator responsibilities include ensuring efficiency in the learning experience for current MSMS students. The MSMS program is fully online. Moderators will attend online classes with MSMS and relay any feedback they may have to the faculty and staff so that necessary changes or adjustments can be made to improve the online learning environment. Moderators ensure that MSMS students have a successful experience as they progress throughout the program, tailoring the experience to meet the needs of students, provide effective communication, and enhance the learning environment for both students and faculty.

Fellows also provide tutoring to MSMS students in all content covered throughout the duration of the program, and help guide students in creating timelines for applying to health professions programs.

The MSMS Fellowship/Moderator program follows an 11-month MSMS hybrid term schedule. The Fellowship begins in the Fall Term and concludes at the end of the Summer Term.

The Post-MSMS Fellowship/Moderator positions are remote. Fellows may report to the SCU campus, as needed. There may be travel to events and conferences, as needed.

Selection process:

The SCU MSMS Fellows are selected based on their potential to make a positive, long-lasting contribution to their academic discipline.

Fellows/Moderators are selected each year through a competitive application process based on their commitment to health education and issues, leadership potential, and interest. Fellows/Moderators should have experience in education, including teaching experience working with diverse student populations. The fellowship requires excellent written and oral communication skills, excellent presentation skills, and the ability to communicate at all levels with tact and diplomacy to achieve goals.

The fellowship has a broad interdisciplinary focus; therefore follows/moderators will have the ability to work across all areas of a complex organization and must demonstrate the ability to work collaboratively with faculty, staff and students on health science education and research.

Only SCU MSMS students who will be completing their MSMS program are eligible to apply. Interested MSMS students should apply for the fellowship during the last term of their MSMS program, or soon after graduation.

The program seeks to achieve racial, ethnic, gender, and discipline diversity. Candidates from underrepresented groups are strongly encouraged to apply. Applicants must be U.S. citizens or permanent residents of the U.S. or its territories who have career plans that anticipate continued work in the U.S. after the fellowship period.

Reflecting on my time as an MSMS student, some of the most important lessons I learned was the value of resourcefulness and what it takes to become a proactive, successful learner both inside and outside the classroom. The SCU MSMS Fellowship allowed me to further build on this experience while sharing my insights with the next generation of students. This unique opportunity enabled me to collaborate as a peer with the very professors who had once taught me, offering a fresh perspective on the program.

In this role, I became an advocate for student voices, contributing to meaningful improvements within the curriculum. Andy Shollar, SCU Class of 2022 MSMS graduate, is a 2023 MSMS Fellow. Prior to earning her MSMS degree from SCU, she earned a Bachelor of Science degree in Biomedical Sciences from the University of South Florida. “My career goal has always been to pursue medicine as a physician. I am currently applying to medical schools this application cycle and hope to begin my journey towards becoming a physician this upcoming Fall. However, throughout undergrad, and in my current role as an MSMS Fellow, I have always found myself leading others from an educational standpoint. I want to integrate my passion for education and medicine by eventually going into academic medicine so that I can pass on my knowledge and experiences to others that wish to follow the same path as me.”

Tiesha Tyson, SCU Class of 2021 MSMS graduate, is a 2023 MSMS Fellow. Prior to earning her MSMS degree from SCU in 2021, Tiesha earned her Bachelor of Science degree in Biochemistry from Coastal Carolina University in South Carolina. “My goal is to become a doctor, preferably a General Surgeon or Forensic Pathologist,” she said.

Beyond the Paycheck: Top Nonsalary Perks for Doctors

Holly Wyatt, MD, had spent 20 years in UCHealth with no plans to leave. Her home, support system, and lifestyle were all rooted in Denver. But in 2020, The University of Alabama at Birmingham (UAB) made the endocrinologist an offer she couldn't resist.

The pay increase and a bump to full professorship weren't enough to lure her across the country. But then UAB sweetened the deal with fewer clinic hours and paid time to create. "I didn't have to fit into the typical 'see patients 5 days a week, bill this many dollars,'" she said.

With no minimum billable hours, she could spend her time on clinical trials, designing programs, and recording podcasts. "When they offered that, I said, 'Ooh, that's enticing.'"

After a couple of visits to the campus, she began the job transition.

Doctors are looking for more than base pay. For many physicians, like Wyatt, nonsalary incentives carry a lot of weight in the recruitment and job-hunting process.

"Some of the usual suspects are CME [continuing medical education] budget, signing bonuses, relocation assistance, loan repayment programs, and housing allowances," said Jake Jorgovan, partner at Alpha Apex Group, a physician recruiting firm in Denver.

Post pandemic, doctors are vying for other benefits, perks that support their interests, work-life balance, and financial stability. "We've come across offers like sabbatical opportunities, paid time for research or personal projects, and even concierge services that handle things like grocery shopping or pet care," Jorgovan told Medscape Medical News .

Amid physician shortages, doctors have more bargaining power than ever.

Money Still Talks

Financial perks are still the premiere portion of a benefits package, according to Marc Adam, physician recruiter at MASC Medical, a medical recruitment firm in Fort Lauderdale, Florida.

New data from the medical staffing company AMN Healthcare reported that the average signing bonus for physicians is $31,103. The average relocation allowance is $11,000, and the average CME allowance is $4000.

"CME budget and loan repayment programs are big because they directly impact career advancement and financial well-being," Jorgovan said. He said that given the high cost of medical training, loan repayment help, especially, has become a huge deciding factor for clinicians. Employers have historically been hesitant to offer these kinds of long-term benefits because of the financial commitment and planning involved, but that's changing.

Adam said that short-term financial perks, like relocation assistance and signing bonuses, tend to be more important for younger doctors. They're not yet financially established, so the relocation support and bonus funds have more impact as they take on a new role, he said.

Mid- and late-career doctors, on the other hand, are less beholden to these types of bonuses. Adam has recruited established doctors from across the country to Florida, and he said that the relocation allowance and singing bonus didn't even rank in their top five priorities. Similarly, in Birmingham, Wyatt recently reread her offer letter from UAB and was surprised to find a relocation stipend that she never used. "I had no idea," she said.

Vying for Time

Mid- and late-career doctors who have a better financial safety net tend to seek benefits that boost their quality of life.

One of Adam's recent job-searching clients was unwilling to compromise on priorities like specific location and a 4-day workweek.

Four-day workweeks, flexible scheduling, and options for remote work are increasingly popular, especially since the pandemic. Some physicians, like those in primary care, are looking for dedicated charting hours — paid days or half-days set aside for updating the electronic medical records. Other doctors are negotiating multistate telehealth licensing paid by their employer and work-from-home telehealth hours.

"Work life has been slowly increasing over the 14 years I've been doing this. And post COVID, the employer's willingness to be flexible with those types of accommodations increased," said Adam.

Priya Jaisinghani, MD, an endocrinologist and obesity medicine specialist in her second year of practice, NYU Langone Health, New York City, said work-life balance can be a priority for young doctors, too. After training in New York during the pandemic, Jaisinghani was all too aware of the risk for burnout. So she negotiated a 4-day workweek when she took her first job out of fellowship in 2022. "I was able to prioritize work-life balance from the start," she said.

Support for the Career You Want

When Jaisinghani signed her first contract in 2022 with NYU, her move from New Jersey to New York wasn't far enough to warrant a relocation allowance. "There was a signing bonus, sure," she said. But what really grabbed her attention were perks like mentorship, access to trainees, and autonomy.

Perks that support long-term growth — like CME allowance, teaching opportunities, or access to leadership tracks — are especially important to young doctors. "After dedicating so many years to medical training, you want to look for some degree of autonomy in building your practice," she said. NYU offered her that kind of freedom and support.

On top of personal growth, young physicians are looking for perks that will allow them to build the practice they want for their patients, Jaisinghani told Medscape Medical News . A lot of young doctors don't know that they can negotiate for schedule preferences, office space, their own exam room, and dedicated support staff. However, they can and should because these factors influence their daily work life and patient experience.

Experienced doctors are also looking for perks that support the career they want. Recruitment experts say that doctors tend to look for opportunities that accommodate their interests One of Jorgovan's recent clients took a position because it offered a generous CME budget and dedicated research hours. Similarly, Wyatt at UAB moved because her contract included paid time to create.

"It really comes down to the need for balance — being able to keep learning while also having time for personal life and family," Jorgovan said.

Making and Meeting Demand

Thanks to the rising demand, doctors have more power than ever to negotiate the perks they want and need.

The existing physician shortage — driven by retiring doctors and an aging patient population — was only exacerbated by the pandemic. Now, a number of new market entries are further increasing competition for talent, according to AMN Healthcare's report. Retail clinics, urgent care, telehealth companies, and private equity firms compete for the same doctors, driving up salaries and doctor bargaining power.

"Physicians were always in the driver's seat, and their bargaining power has only increased," Adam said. Healthcare systems, once reticent about flexible working arrangements or loan repayment, are reconsidering.

Even young doctors have more negotiating power than they realize, but they might need help. "It's underrated to get a contracts lawyer as a young doctor, but I think it's smart," Jaisinghani said. They're often more familiar with salaries in the area, flexibility options, and potential benefits, none of which doctors are taught in training, she said.

Adam said that the pandemic opened employers' eyes to the fact that doctors have the bargaining power. There's a stark need for their talent and a lot of public support for their service. So hiring managers are listening and are ready to offer "creative benefits to accommodate the market demand," he said.

In her new position at UAB, Wyatt said that money will always matter. "When your salary is low, bumping that salary will make you happier." But after a certain point, she said, other things become more important — like your time, the work you do, and the people you work with. Her perks at UAB offer more than money can. "I get up in the morning, and I'm excited — [the work] excites me," she said.
