The Johns Hopkins speech-language pathologists (sometimes informally called speech therapists)  provide therapy for adults whose ability to speak or swallow has been affected by an injury or illness. Our speech-language therapists assess and treat a variety of speech, language, cognitive and swallowing disorders. This may include retraining for “slurred speech” after a stroke, cognitive therapy after brain surgery, swallowing therapy related to a neurological condition and everything in between.

Common Conditions We Treat

Swallowing disorders.

Swallowing is a complex neuromuscular activity and is susceptible to problems as a result of strokes, head injuries, esophageal disorders, head and neck cancers and neurological diseases like Parkinson’s disease.  Swallowing disorders (dysphagia)  are of particular concern and urgency because in its severe form, dysphagia can lead to aspiration pneumonia.

Our speech-language pathologists offer clinical evaluations and diagnostic tests to assess the swallow function. We work closely with patients and families to tailor a plan that can promote good eating and drinking habits while preserving health and safety.

Difficulty Speaking

Difficulty speaking often has to do with problems in engaging muscles involved in speech (apraxia). Speech impairments in adults are most commonly the result of a stroke, head injury or brain surgery.

Treatment focuses on strengthening the muscles used for speech, improving coordination and training in strategies for improving intelligibility of speech.

Language Problems

Neuro-cognitive issues.

Speech-language pathologists also evaluate and provide rehabilitation for cognitive deficits, which are most often associated with strokes, head injuries and traumatic brain injuries.

Therapy may include learning strategies in improving attention, memory function, organization, reasoning and problem solving, with the primary objective of returning patients to safe and functional living while maximizing independence.

Voice Problems

Voice is assessed both perceptually and objectively using diagnostic tools, such as a computerized speech lab. There are many causes for voice disorders (dysphonia). People who use their voice for their employment are often at risk for straining the vocal cords and/or developing nodules or polyps. Other conditions that affect vocal function include spasmodic dysphonia, presbylaryngis, cancers and Parkinson’s disease, to name a few.

Voice therapy may focus on education in vocal hygiene and training in reducing vocal strain and developing a strong and clear vocal quality. Our speech-language pathologists work closely with the referring physicians to develop a treatment plan that is effective and appropriate for vocal rehabilitation.

How do i find a speech and language therapist, how do i find a speech and language therapist  .

There are two main ways that you can find a speech and language therapist:  

  • Via your local NHS service  
  • By contacting an independent speech and language therapist  

Local NHS services  

You can find your local NHS service by doing an internet search. Try searching ‘NHS Speech and Language Therapy near me’. Most local services will have their contact details available, and information about how to refer your child.   

Do I need to go to my GP or health visitor?  

It’s best to check with your local service first. Many services will accept referrals for children under five directly from their families. Children over five may need their school to make a referral. Some services may offer drop-in sessions locally at children’s centres or online, while others may need a referral for an initial appointment.  

How do I find an independent speech and language therapist?  

There is a searchable database of independent therapists who are all registered here. You can search to find a speech and language therapist by location, age, and the type of support your child needs. You can then contact the speech and language therapist directly. They will be able to give you more information about the types and costs of the services they provide, and their availability,  

I have health insurance – will this cover speech and language therapy?  

Some health insurance providers do cover speech and language therapy. They may have recommended or registered providers. It’s best to check your policy first.

How to find a speech and language therapist 

There are a number of ways to access speech therapy or a speech and language therapist (SLT), depending on whether you are looking for an NHS or a private (independent) SLT.

To access speech and language therapy services through the NHS, ask your GP, district nurse, health visitor, your child’s nursery staff or teacher for a referral.

You can also refer yourself to your local speech and language therapy service. Ring your local NHS service and ask for the telephone number of your local NHS speech and language therapy service.

You do not have to wait for someone else to refer you.

How to contact your local NHS

  • England – please contact your local GP for advice.
  • Northern Ireland

You can also find the number of your local NHS service online or ask at your GP surgery.

What happens next?

This varies across the UK because services are organised in different ways. In some places, demand for services is very high.

Some areas run a system where first referrals are sorted before appointments are made.

If you have this system in your area, a speech and language therapist or assistant may telephone you first to find out more about your situation. At this stage, you can ask what will happen next and how long you may have to wait for an appointment.

If you think you have been asked to wait too long for a first appointment or for treatment after the first appointment, contact the speech and language therapy department to ask what has happened.

If you still experience difficulty, contact your local NHS service to discuss the situation.

Independent speech and language therapists can usually offer an immediate appointment for assessment followed by therapy to suit the client. You can find private SLTs through the Association of Speech therapists in Independent Practice (ASLTIP).

Visit the ASLTIP website to search for a local SLT by postcode, age range, languages spoken and conditions treated.

Some independent therapists offer specialisation in specific areas of communication difficulties, including:

  • Assessment and diagnosis of complex disorders sometimes in association with other professionals.
  • Tutorials for specific problems, such as dyslexia.
  • Second opinions and reports for statements of special educational need and attendance at tribunal assessments, reports and court attendance for medico-legal claims.

What is Speech Therapy and How Do I Find a Speech Therapist?

Last week, we started discussing some of the basics of speech therapy, including why your child might benefit from it. This week, we’ll continue the four-part series, covering questions such as “How do I find a speech therapist?” “How do I do at-home speech therapy?” and “Just what the heck does a speech therapist do, anyway?!?” Whether your child has recently received a diagnosis or you suspect that he might have a speech and language issue (or you just want to brush up on the basics), check out these introductory guides. What Can Speech Therapy Accomplish?

As we discussed in last Friday’s post , speech therapy can accomplish a wide range of objectives for your child. Speech-language pathologists (SLPs) help children improve their receptive language skills (the ability to process information) and expressive language skills (the ability to express themselves verbally or nonverbally).

To meet those goals, the SLP might work with the child to build his vocabulary, understand the rules of grammar and how to form proper sentences, and improve his articulation with the use of Speech Buddies and other tools. The specific areas that your child’s SLP will work on and the tools she’ll use will depend on your child’s specific needs. No two children are alike. Johnny might need help pronouncing his “s” sounds, for example, while Micah has trouble organizing information and following multistep directions. Your child’s SLP will evaluate your youngster, determine his needs, and formulate a customized treatment plan that can help him.

How Can I Find a Speech Therapist?

Depending on where you live, you likely have many options for finding a speech therapist. (Rural areas often do not have as many ASHA-certified SLPs , but even if this is the case, you may still be able to access a highly qualified speech therapist via telepractice. ) For starters, you may need to decide between public or private speech therapy.

Public speech therapy refers to free speech therapy services that are offered by your school district. Public schools are federally required to provide all children with a free, appropriate public education, and if your child is found eligible for services, he can receive speech therapy in this manner. If your child is not found eligible for free services, you can enroll him in treatment with a private speech therapist who works out of a hospital, clinic, or private practice. (You might also combine public and private speech therapy if you’re less than satisfied with the extent of the services offered at the school.)

How Can I Find the Right Speech Therapist for My Child?

Finding a speech therapist is typically not a simple matter of finding a name in a phone book and scheduling treatment sessions. You need to make sure that she is the right SLP for your child. Think of the process as being similar to job interviews. Some candidates are a great fit for the position, while others offer a skill set and experience level that just don’t quite fit. Meet with several SLPs, with and without your child, and observe how your child interacts with the SLP (and vice versa). Your child should feel comfortable around his SLP, or he might not put much effort into the speech therapy. Here are some factors to consider and questions to ask:

  • Has the SLP been certified by ASHA?
  • Does the SLP have a current state license?
  • Does the SLP have extensive experience in treating your child’s specific speech disorder?
  • Does the SLP have experience with other special needs that your child might have (i.e. autism, Down syndrome)?
  • Does the SLP accept your health insurance?
  • Will the SLP keep in touch with you on a regular basis and provide written progress reports?
  • Does the SLP encourage you to do at-home speech therapy techniques with your child? (This can greatly accelerate your child’s progress.)

About speech and language pathology.

The Speech-Language Pathology team provides services to NIH patients with oral motor, speech, language, voice, cognitive, and swallowing language and other cognitive impairments. Goals are directed towards optimizing function and enhancing quality of life.

Clinical tests are performed to assess the effects of patients' impairments on functional communication or swallowing in order to determine the best course of clinical management and effect positive outcomes. Specialized studies are conducted to objectively assess physiological function of the oropharynx, and to assess and treat language and communicative discourse abilities in neurologically impaired populations. Infants and children with various genetic or developmental conditions are assessed to establish accurate diagnosis and to provide clinical recommendations to local programs.

Staff members serve in both collaborative and independent clinical research capacities. Research is conducted in swallowing, voice and language and discourse functions. Research studies use brain mapping procedures, on-line cognitive measures and video-fluoroscopic imaging along with computerized voice analysis and stroboscopy to assist in understanding changes caused by medical, neurological or genetic conditions.
